Once the patient has signed the medical statement form, it is then sent by the hospital to your doctor who will be handling the case. The doctor will give instructions about the treatment that the patient can receive, and also about the hospital environment where the patient is going to be treated. Once the physician has obtained the medical statement form and all of the documents from the patient, then he'll send them back into the hospital. The hospital will then confirm if all the documents are signed and send back it to the individual.
