Each resident must complete the Northern Mariana Islands Health Declaration Form in three steps before they can start a new medical condition. You can fill out this form in person, online, or by phone. It will inform the residents of certain types of health services they are entitled to receive from the Northern Mariana Islands, as well as other information they should know regarding the rights and responsibilities of an island resident. The three steps include a description of the medical condition, the list of conditions, and the procedures that will be administered for each condition. Some of the documentation required for each step can also be completed online. However, the entire process usually takes place in person.
