The Bonaire Health Declaration form is a five-page document that you must complete before you travel to the island. The form is very convenient for those traveling to Bonaire because it can easily be filled out on the spot and then turned in at the border before leaving the country. This form allows the Embassy or Consulate of your destination to view your medical history and determine if you have any severe health conditions. This form can be used to help prevent an emergency situation, such as an import illness, becoming a concern for you and your family.
The Bonaire Health Declaration Form consists of five pages, which you need to follow carefully. The first page contains information on the name and birth date. The second page contains your address, where you live, and where you work. The third page includes the last four digits and area code of your telephone number. The fourth page consists of your social security number, plus your date of birth if you do not have one.
The fifth and final page consists of your medical history, including details about your family, your recent travels, and your current health condition. Even if they are minor, you must list all of your past and current ailments. You also need to detail your current medications and if you are taking any over the counter drugs. You need to give details about your blood type, whether you are diabetic or whether you are HIV positive. You must also describe any travel that may have affected your absorption of food substances or other allergens. If you are travelling with children, then the document will need their name and the first name of a qualified physician, preferably a pediatrician.
