Bonaire Health Declaration
If you are applying to financial assistance or a Bonaire resident, you must complete the Bonaire Health Declaration form. Although there are no specific medical requirements, you must ensure that you are in good health and are free from any significant health problems. A pamphlet explaining the process and providing instructions for filling it out is also available. Once you have filled out the form, you can't make any changes. You cannot change the documents once they have been submitted.
