Bhutan Health Declaration Form
The Bhutan health declaration form is basically a review form that collects basic information from visitors coming to the country for medical care. It includes both foreign and national patients who are coming to Bhutan for medical care. The document must be completed by foreigners before they can leave the country and then enter the country to receive medical care. The form is not required for residents of Bhutan to register for any type or medical insurance. It is also needed for applying for passports. This form must be presented to government officials upon arrival in Bhutan. This is also used to prevent the fraudulent claims of medical care from gaining financial support.
Before you set out to complete your bhutan health declaration form, you should be aware of the different documents you will need as well as the process of getting a positive certification. One of the most important requirements is that you must provide a negative certification which states that you have not undergone any surgical operations in the past two years. Foreign tourists who do not posses a negative certification must obtain a separate registration. Bhutan does not accept requests from tourists who do not posses a negative certificate.
