Form for Belgium Passenger Locator
The Passenger Locator Form is an online tool that is required of all travelers entering the country. The Passenger Locator form must be completed at least 48 hours prior to arrival. The Passenger Locator Form is available at no cost on the FPS Public Health Website. To submit the form, you will need a valid email address and a mobile phone. It is recommended that you do this as early as possible, but there is a deadline of one month before arrival.
