The Spain Health Declaration Form (ilingual), which is legally binding, outlines the medical conditions currently covered by the Spanish social security system. Anyone who seeks medical treatment in Spain, or any Spanish resident, can use the form. The Spanish national health system covers all types of people and provides comprehensive coverage for medical services as well as diseases. The National Health Services (SNH), which is the declaration form, contains detailed instructions for filing an application for any type of Spanish medical insurance.
To enroll in the Spain insurance scheme, you will need to complete the declaration form available at the SNH office. This is usually done on an easy to understand letterhead that can be printed and provided for the recipient of the form. The first three pages of this form provide general information about the applicant's current health, past medical issues, and family history of diabetes. The fourth page contains a separate section where the applicant can declare the kinds of insurance he or she wants to avail of, in particular, health insurance that covers dental care.
