Once you submit your application, you will immediately receive a printed copy of the Bahamas Travel Health Card along with all the required information regarding the application. The applicant's name, date of birth, mother's maiden names if they were born before 1950, and the full legal name of his or she father, as well as his or their residence address. Also included are his or her physician's names, years of practice and the National Association of State Boards of Medical examiners' page where he or she has been certified.

You must also present the Bahamas Travel Health Card and a scanned copy of your passport. These three documents must be presented along with the payment of a fee of up to $75. If you have a Bahamas Travel Health Card, but no passport or driver’s license, you must submit these documents together with your payment within one month of receiving it. Otherwise, your card will be rejected.
