Improving Diabetes Care and Outcomes on the South Side of Chicago
At our organization, we strive to provide top-level care for diabetes patients. Through various quality improvement initiatives – including group visits, peer support groups and patient registries -we have been able to redesign clinic operations in order to better meet the needs of this vulnerable population. We are also looking into new approaches such as employing community health workers/patient navigators and coordinating specialist access; thus ensuring that all necessary resources remain within easy reach of those who need them most.
