The electronic health declaration form for Hong Kong can be collected from any of the clinics and hospitals that are registered with the Department of Health. You do not have to send the document to the Central Government Office as they will handle the document delivery. If you wish to apply for a medical examination or treatment, you have to fill in the electronic health declaration form and send to the Department of Health. The document will be sent to you within 15 days.
