You must complete a Travelers Health Declaration form before you travel to Brazil. This form can be completed online and is required for all travel to Brazil. After you know the current entry restrictions and have a negative PCR or antigen test, you can begin planning your trip. You can complete the form online, but it must be signed by an official from the Department of Foreign Affairs. This information must be provided if you are a US citizen before you travel to the country.
